I agree with an earlier nomination of Yoko Kanno (Macross Plus,
Escaflowne).

However, if I can be allowed to deviate from the original premise, I
nominate Madoka. She comes from a musical family, sings, plays, and
composes. As we see in Shin Kimagure Orange Road, her music transcends
time itself.

My favorite though is the song she sings in the Heart on Fire OAV.
