I have always been confused about the Geofront? What exactly is the Geofront?
Is there a city above the Geofront? I always thought there was but episode 19
would imply that Tokyo-3 is actually the geofront. If there is an actual city
above the geofront, how many angel battles take place in the geofront?

A Geofront is, more or less, an underground city. Shirow also used the concept in the Ghost in
the Shell manga. In the case of Evangelion, the Geofront is where NERV HQ is.

Yup. That would be Tokyo-3 (or at least *a* Tokyo-3.)

This is where I'm confused. If it weren't for the watermelon patch, which seems to be in Tokyo-3
and the Geofront simultaneously, then I'd say pretty much all battles took place above ground.
Although TEoE confuses this even more when the N2 bomb exposes the Geofront, which clearly shows
that there's only forest, a lake, and NERV HQ in there. *shrug* I dunno. Just another one of
those nagging continuity/consistency problems with EVA, I suppose.

The Geofront is a large subterranean cavern. Tokyo-3 both is and is not
the Geofront. The civilian buildings on the surface retract through the
Geofront and hang from the cavern ceiling when the city goes into seige
mode, extending when danger is past and repairs are finished (ep. 1-2).
The lower surface of the Geofront consists only of a nature area, NERV
headquarters, and the lake. All of the emergency bunkers are also
located in the Geofront, as it is ostensibly the safest place to be in
the event of an angel attack. This is why Shinji ends up in the Geofront
after abandoning the shelter in ep. 19. Apparently, the Geofront also
has spectacularly effective lighting and sprinkler systems, which allow
its surface to sustain grassy fields and a coniferous forest.
The locations of angel fights are as follows (IIRC):
3rd--city surface
4th--city surface
5th--city surface
6th--ocean
7th--ocean/city surface (2 encounters)
8th--volcano
9th--city surface (tries to penetrate to Geofront with acid)
10th--space/city surface
11th--Geofront (NERV HQ)
12th--city surface
13th--surface
14th--Geofront
15th--space/city surface
16th--city surface
17th--Geofront (NERV HQ)

The only problem with this (and it's not your fault, its Gainax's) is that
the shelters/bunkers
are clearly on the surface as seen in eps 4 and 6. (4th & 5th angels.)

Well beyond our technology at least. If you noticed, there's also a sun,
moon, and stars projected in Geofront.
