item/materials request form (DRAFT-please review)

Hey everyone!

I've been working on this for awhile now and it's finally time to share it in it's draft format! (I will "pretty it up" later, wanna get the functionality settled/completed first.) 

Folx/organizations/artists can use this request form to either request items from REBUILD or to offer items up that they have in excess. This is the beginning of a larger organizational project to attempt to get many of these details outta my brain and written down somewhere where they can be accessed by others, without having to go through me. Another step will be for the data to be viewable, so volunteers with REBUILD can check to see what is needed at any given time. 

Please review it if you can and let me know if you see any glaring omissions or mistakes. The form is "live" so if you hit submit, I will actually get whatever data you've input. (I can delete it after, if you're just testing)
