Inventory import csv layout for Fetian C-200 tokens

We have a csv file with our Feitan C-200 tokens which has the token number and secret. We do not have a PSKC file only a native tnk file.

Thus we cannot convert using the pskc2inv.
What is the layout of the csv file that the inventory file import is expecting?

For example:

Type, Unique id, Secret

Fetian c200,2301230423,s80sd8s08s08 

Or similiar.
