Hi Dave,
The RC2040 and the Pico SD Card Module are different things from
ExtElec. You should be looking at this repo;

When I supply these as a full kit the Pico comes pre-flashed, but
if you bought a PCB only, then you'll need to use the .uf2 file in
that github to flash your Pico.
You're right that the /HALT signal does need to be connected to the bus. On RC2014 Standard Bus computers, like the RC2014 Mini, this would be via a flying jump lead. On RC2014 Enhanced Bus (And probably RCBus) computers the /HALT signal is in the 2nd row of pins so nothing else needs to be done.
Hopefully that gets you on the right track.
