Creating groups based on user input

Hello, 

I need help regarding creating groups without manually creating one. I am registering users and they enter their regions and districts. I would like to create a group on spot within the flow if it is not created and then add the user to it depending on his response. 

for example;

1.  when a user enters his region as Elgon, A group is created if it never existed and the user is added to the group. 
2. likewise when he enters his district as Elgon, he is added to a group Elgon. And if the group never existed, it is created. 

Any help is highly appreciated
