Your family's immunization records refers to records for a person you are the parent/guardian of. You cannot request a record for your spouse/partner unless you are their legal guardian.
You can access an immunization record for you or your child one of two ways. You can download an app called Docket where you can download and print your own PDF copy of a record OR you can request a copy of a record by submitting a record request to the MIIC team. If you need to update outdated personal information (your name, address, or phone number) in MIIC, please complete a MIIC Record Update Request. MDH staff will review the form and update the contact information in the MIIC immunization record. If staff are unable to find a MIIC record, they will contact you. Requests will be processed in the order they are received.
Sign up for MyIR Mobile by going to myirmobile.com and follow the registration instructions. Your registration information will be used to match your records with the state immunization registry. You will be sent a verification code on your phone to finalize the process. Once registration is complete, you'll be able to view your immunization records, Certificate of Immunization Status (CIS) and access your COVID-19 vaccination certificate.
Most healthcare providers in Washington use the Washington State Immunization Information System. Please check with your provider to ask if they can give you a complete immunization record for you or your child. They can print it from the Immunization Information System or from their own medical record system.
If you can't get a complete immunization record from your provider and you don't want to sign up for MyIR, contact the Office of Immunization at 360-236-3595 or 1-866-397-0337. The Department of Health uses the Washington State Immunization Information System, but it does not have complete immunization records for all people. When you call, you'll be asked to provide identifying information, such as name, date of birth, and address. At that time, we will tell you if a record exists in the system, but no other information will be released over the phone. This is to protect the privacy and confidentiality of your information.

If you are not sure if you have already uploaded your vaccination record, simply log into MyChart and select "Immunizations" from the left-side menu. (Click the three white lines in the corner to expand the menu.) If you see thumbnail graphics representing files, such as a PDF or a photo, you have already uploaded your record.
SHCC Staff will review, verify, and acknowledge the immunization record. After that process, which may take 2-3 business days, you will receive a secure message in MyChart from the clinic staff confirming that you are no longer required to participate in surveillance testing.
If you were vaccinated elsewhere in New Jersey, we can search for your vaccine records in the New Jersey Immunization Information System. Expand the section showing your vaccination status and click "Update."
Record your COVID-19 vaccinations/boosters in MyChart and then print or download your vaccination records via a QR code or as a PDF to keep on file or present as proof of vaccination. You may access the same records in this area for any proxy accounts you have access to, such as a child.
The Personal Notes feature allows you to add your own secure notes about your healthcare right where you need them most. This is designed for your own record-keeping and is not a way to communicate with your provider(s). Personal notes can only be viewed by you and are not part of your permanent medical record.
Your provider might request that you record information such as your daily glucose or blood pressure readings in MyChart. The order is called a "flowsheet" in MyChart. When you enter the information in the flowsheet, your provider automatically receives the readings so he can monitor your progress between visits.
Share Everywhere allows you to share your record with just about anyone, like a doctor, chiropractor, social worker, physical therapist, dentist, or school nurse. Grant someone one-time, limited access to the health information you can see in MyChart, including health history, medications, allergies, immunizations, etc. This gives him or her the ability to see your health record summary, view other clinical data, and submit a note to your care team. You can use MyChart to view your health information from other organizations where you and your family have records. When you connect your accounts from the participating organizations, you will be able to see the following types of information in your MyChart account:
If you have access to your family members' medical records, you can view most of the information in their records in the same way that you view your own. Some things that might be useful include viewing a family member's test results or your child's immunization records.
To access a family member's record, you first need permission. After you've received permission, you can view their records by hovering over the icons in the top toolbar and clicking on the appropriate record.
When you are in your child's record in MyChart, go to Your Menu > Health Summary > Immunizations. You can see the immunizations your child has received and the dates on which she received them. Click the immunization name to learn more.
If you've lost your vaccine card and need documentation indicating that you received your COVID-19 vaccine, you can access your record through MyChart. To have your record sent to a third party, complete the Wisconsin Immunization Registry Record Release Authorization, F-02487. If you, a parent or guardian locked your record in the past, you can use this form to unlock your record. The record must be unlocked for you or your health care provider to access it.
People who receive immunizations from South Carolina providers can access their immunization record through the SIMON public portal. As of Jan. 1, 2017, all immunization providers are to report all administered doses of vaccines to the state immunization registry. If you have questions regarding your immunizations, please reach out to your primary health care provider or the provider that administered the vaccines.
Anyone at least 18 years of age with a vaccine record in IRIS can use the Docket app to review their own immunization record or to review the immunization record of someone for whom they are the parent/legal guardian or authorized personal representative, usually for a minor child or vulnerable adult.
Docket displays all your immunization records that are entered into IRIS by your healthcare or vaccine
provider. There are various reasons why your complete immunization history may not be in IRIS; please check with your healthcare provider if your record is not up to date.
A Docket user must be at least 13 years old to create an account, however users in Idaho must be at least 18 years old to search for records. In addition, you must be at least 18 years of age to access your IRIS record through Docket or via a Record Request to the Department of Health and Welfare.
Once your child is a legal adult (on their 18th birthday), you will no longer be able to access their record. However, Docket makes it easy for your child to access their own record. Other ways for your adult child to obtain this information are to request them from their physician or by submitting a Record Request to the Idaho Department of Health and Welfare.
Your MIIS record also contains all other immunizations you have received that were reported to the MIIS by your Healthcare Provider. Click Print All Immunizations to print a copy of your immunization history.
If you received your vaccination from a federal agency (like the Department of Defense, Indian Health Services or Veterans Affairs), you will need to contact those agencies for assistance with your vaccination record.
If the information on your vaccine record is incorrect (e.g. missing dose, wrong dates or incorrect brand), you may need to correct or update your immunization record. You can contact your healthcare provider to update your record or submit an Electronic Amendment Form. You will be notified of our findings and remediation actions within 2-3 weeks.
Once a provider submits an update, it will take 24-48 hours for the update to be visible on your record. If updates are made to your COVID-19 vaccines, you will need to access an updated SMART Health Card through the portal 24-48 hours after your healthcare provider has submitted the update.
As life gets back on track for many families, some activities, such as concerts, sporting events and travels abroad, may require proof of COVID-19 vaccination for the health and safety of everyone. Rather than relying on paper CDC vaccine cards that can get lost or left behind, MyChart is making it easy with digital vaccination records available at your fingertips.
