Supply voltage problem, maybe?  Try a different socket on the host, or a powered USB hub in between the Quadstick and the host.

It might be worth trying a different USB cable.  I have one here that causes all the sensors to read a soft sip if I move it just right.

I also tried my old QuadStick and it's doing the same thing so not sure why. I tried both new and old firmware and and cleaned the tubes. Any ideas about what could be going on?

My new Quadstick keeps having the same issue as the other 2 with the left puff getting stuck repeatedly tapping even with new mouthpiece and on both my desktop and laptop whether I use USB ports or powered USB hub. Also tried many USB cables. The same issue being hardware related on three Quadsticks seems unlikely and both my laptop and desktop having the same problem seems unlikely too so not sure what to do. It happens on any game profile too unless I avoid mapping left puff to anything.

I don't know what to tell you.  If all of them do it, the only thing in common is their configurations and the hosts they are connected to.  If this happens with two different hosts, and multiple cable/hub arrangements, that would seem to eliminate them, unless the triggering event is something in common between the hosts.

Let's go back to what you observe:   How do you know the left puff is randomly coming on?  Just the #2 red LED, or does the host respond to whatever output the left puff is mapped to?

Changes in USB status can cause the red leds to briefly display the USB connection state.  Whenever the USB connection status changes, the red leds will display it.   Numbers 2 & 4 will light up when the USB status is "OK"

On PCs, disable Selected Suspend for the USB:  http://quadstick.s3.amazonaws.com/documents/user_manual/um/windows_10.htm  Windows could be trying to put the Quadstick to sleep and then realizing it can't do it.  

Switch to USB mode 6, or a game controller mode to remove the Flash drive USB function that Windows tries to suspend to save power.   http://quadstick.s3.amazonaws.com/documents/user_manual/um/usb_emulation_mode_6__disable_flash_.htm

Try restoring your Sip/Puff pressure settings to the default values;

sip_puff_threshold_soft8
sip_puff_threshold40
sip_puff_maximum70

I think you currently have them at 15, 30, 70.

I've mentioned before that clogged tubing or changes in supply voltage can cause sip/puff behavior to change, but you've eliminated that.  While one sensor might be more sensitive to supply voltage than another, it would not be the same sensor on three different quadsticks.  I think voltage thing is not it.

The 2nd red light comes on and the key left puff is mapped to keeps getting activated over and over. Sometimes rebooting works to prevent it. It seems to happen more when turning the PC on and getting on after having it off all night. The USB selective suspend being off seems to be helping now.
