UFT 11.5 for Oracle forms

When i am trying to use UFT for Oracle forms (Versions are supported i have checked) I am getting following error and as mentioned in error message i have tried all types of permutation and combination of installing
JInititor before and after QTP/UFT.
Has anyone face this issue before.
