Clinician Portal-Based Report Submission Code Layout/Example

We have some clinicians who will be submitting their QPP reporting themselves via the portal rather than through our CEHRT and API. We want to ensure that the report we build for them to download is formatted correctly. 

• At present we have EHR-submission focused code, but want to ensure that it is the same format for the clinician initiated portal based submissions.
  

• Is there a good concrete example or code that can be provided for us to review?
