Can you explain how a small group can submit their own quality measures via qpp.cms.gov ? I see that on the site it says that “this upload tool accepts properly formatted QPP Format and QRDA-3 files.” The proper format IS QRDA-3 files correct? Is there a walk through video on this format? We are trying to avoid paying a registry. I am not looking for a video resource of actually submitting the file- I'm looking for how to get my excel spreadsheet in to an acceptable format for CMS.
For a little more understanding, we are a small group with less than 15 providers. We have our measures chosen and we have identified our patients within those measures based on reports our EHR built for us. However, we had to manually abstract who met the numerator, who was excluded, etc. We have all this data in a spreadsheet. How can we aggregate this data from the spreadsheets (excel) in to acceptable CMS format to submit to QPP? We have already completed our IA and we assume ACI will reweight to 0% once we submit our quality data considering we are a small rural practice with less than 15 clinicians and all visits are hospital and ED based.
