Clarification on Single performance rate submission / Calculation

Can someone clarify me with the measures ( Quality Id # 441,5, 8, 143,438)  that have multiple criteria , can we submit the performance rate for both the criteria together or should be done separately.

For Instance, Quality ID #441: Ischemic Vascular Disease (IVD) All or None Outcome Measure (Optimal Control)  has TWO SUBMISSION CRITERIA:

1) Patients with coronary artery disease (CAD) or a CAD Risk-Equivalent Condition 18-75 years of age and alive as of the last day of the measurement period and a minimum of two CAD or CAD Risk-Equivalent Condition coded office visits with a Primary Care Provider (PCP) / Cardiologist in 24 months and one office visit in 12 months

2) Patients with one Acute Coronary Event (Acute Myocardial Infarction [AMI], Percutaneous Coronary Intervention [PCI], or Coronary Artery Bypass Graft [CABG]) 18-75 years of age and alive as of the last day of the measurement period) from a hospital visit (excluding Emergency and Lab Only visits) and have been seen by a primary care provider (PCP) / Cardiologist for two office visits in 24 months and one office visit in 12 month

Does OR implicate either / both the criteria satisfies the measures and can be submitted as one?

For MIPS CQMs the performance rate cannot be submitted and is calculated by the API. I would recommend following the flow in the measure specification, since each of those measures have a different assessment being evaluated. For measure 441, although there are 2 eligible populations, the numerator is tracking the same information. Measure 005 contains strata but is a single performance rate where all performance for patients 18 - 75. The specifications, especially for MIPS CQMs in the green box at the bottom of the flow, outline the specifications. Please let me know if I can provide any additional information.
