QPP Participation tool seems to show some incorrect practice addresses

Hi,

Our registry is asking clinicians to indicate which practice(s) they are reporting for based on the data on the QPP Participation tool in order to determine if they need to report for MIPS or not under a certain practice.

Some providers are indicating that the practice addresses showed at https://qpp.cms.gov/participation-lookup don't match the official addresses for the TIN.

Initially we assumed that some providers changed the address but never updated the official CMS PECOS address. 

But we have examples where the official PECOS address for a practice doesn't match the address showed on the participation tool.

Some addresses seem to match the hospital where the provider works.

For example:

https://qpp.cms.gov/participation-lookup?npi=1023070075&py=2018 belongs to practice RYE BROOK CARDIOLOGY & VASCULAR MEDICINE PC

The QPP Participation tool has this incorrect address: 5 PERRY RIDGE RD, GREENWICH, CT 06830

This CMS database has the correct address for the practice:

14 RYE RIDGE PLZ STE. 234 

RYE BROOK, NY 10573-2826 

From which CMS database is QPP participation tool extracting the practice addresses?
