QPP portal QRDA-III submission issues -- unexpected "no high priority measures" and "less than six measures"

Hello everyone,

I work as a software engineer for an EHR vendor, implementing our eCQM calculation system and QRDA file export system.

One of our customers is having trouble with his QPP submission, and we can't figure out what's wrong.

When our customer uploads a QRDA-III file to the QPP portal, the portal accepted the upload without issue, but doesn't seem to actually acknowledge the data it contains. His portal dashboard displays two errors under the Claims Submission Summary:

  • "No High Priority Measures" - there was no high priority measure in this submission... This submission received a zero for the missing high priority measure.
  • "Submission Less Than 6 Measures": This submission... has not qualified for Eligibility Measure Application. The submission was scored on the measures submitted and received a zero for required measures not reported.

As far as I can tell, these error messages shouldn't apply. The file he submitted had six measures of his choosing, and five of those measures are ones considered "High Priority". He has a non-zero score for every measure he's submitting, which is reflected by the population counts in his QRDA-III file.


I can't find anything wrong with his file, or understand why the data he submitted in it is being ignored.


The QRDA-III files we generate appear well-formed based on every validation tool I know about: 

  • they adhere to the QRDA-III specification (and validates against the .xsd files included with the 2017 implementation guide)
  • they validate against the Cypress Validation Utility (which I'm running in a VM) when generated from the Cypress Gold Standard Test Data.
  • they validate against the QPP Conversion Tool, and will convert cleanly from QRDA-III -> JSON, with all the expected information intact for any included measures.
  • File uploads by our customers to the QPP portal are accepted.

However, for this particular customer, the uploaded data isn't reflected in his score.


Does anyone know what is causing these Claims Submission Summary errors? Can anyone point me to an additional tool I can use to help us diagnose the problem? Why won't the QPP portal recognize the data in our QRDA-III file?


Is there something we're not including in our files? Or could it be as simple as finding a "recalculate" button on the QPP dashboard to clear out previously cached submissions' caclulations?
