Submitting true for 2 unique public health or specialized registries

My question is if a clinician/practice is submitting to 2 unique Public Health Registries (ACI_PHCDRR_4/PI_PHCDRR_4), how would they represent this in the json? Would we just include the measure twice in the set, both with a value of "true"?

The final rule says:

"We are also finalizing that eligible
clinicians can earn 10 percentage points in their performance score for reporting to any single
public health agency or clinical data registry to meet any of the measures associated with the
Public Health and Clinical Data Registry Reporting objective (or any of the measures associated
with the Public Health Reporting Objective of the 2018 Advancing Care Information Transition
Objectives and Measures, for clinicians who choose to report on those measures) and, and will
award an additional 5 percentage point bonus for reporting to more than one. "
 
"a MIPS eligible clinician may earn the bonus score of 5
percentage points for reporting to at least one additional public health agency or clinical data
registry that is different from the agency/agencies or registry/or registries to which the MIPS
eligible clinician reports to earn a performance score. A MIPS eligible clinician would not
receive credit under both the performance score and bonus score for reporting to the same
agency or registry."
