Group Reporting

Couple of QPP JSON questions on group reporting.

1) With the group do you report each provider in the group or do you add up all of the measures for the group as if it was a single provider?

2) With a group that is mixed specialty that is reporting on different sets of measures,  would you do a measure set for each specialty in the group?

3) If a particular measure is done by multiple specialties (ie overlap), does each specialty have to do the measure?  for example #128 BMI screening.  If a patient sees a Physcial Therapist in January, and then returns in August and sees a Occupational Therapist do they have to redo the BMI screening since it is now a different specialty.  The answer to #2 might clear this up a bit too.
