One thing to check is that all the inputs have level. (Left Hand Column). QLab will present the correct number of channels when a cue is dragged in but the input levels will be as in your default setting in settings/audio.

Actually, there is a reason this isn’t working; it’s the distinction between channels and tracks in video files. QLab supports multiple channels of audio, but only one audio track.
That is to say, Mic’s example, which works, has a single audio track that contains multiple channels. Mike’s file has multiple audio tracks, which will not work. His first track is stereo, which is why the first two channels are showing up in QLab.
