On the first two questions you asked:
OUT reflects the thresholds you defined in your analysis (consistency, possibly also frequency, PRI). Any row that meets the thresholds is coded 1 on OUT. But this doesn't mean that the cases that show membership above 0.50 in that row also show the outcome. It may well be that some cases don't show the outcome and the row is still used for the minimization (which will lead to deviant cases). It may also be that some cases that show the outcome are assigned to rows that are coded 0. This would lead to unaccounted cases in your analysis.
