FOR ALL residents.. AS i told all of you before,  this is the faculty evaluation form, and it must be filled by each one of the resident in order
to evaluate the faculty members and their performance.. consider the following:
1. the resident can fill more than one form if he/she want to evaluate more than one member.
2. the resident can evaluate good and bad members as well.
3. the resident can evaluate all the faculty members including the  program director, assisting program director and chief resident.
4. 100% CONFIDENTIAL feel free, evaluate whatever u want, by whatever u think it is fair.
5. submitt your evaluation to dr.azhar or dr.baha.   (if u trust me, i will be more than happy to collect your feedback +/- forms )

Majid this is an excellent step, But evaluations of any individual should never go back to the evaluate.
