I'm not sure if I understand exactly what you are asking. Are you talking about creating a new database to store patient information? I know there are some projects that keep databases of things like DVHs and contours to build up reference data sets for IMRT planning. I'm not sure if they are open source or use python, though.
If you are talking about editing a clinical database like Aria or Mosaic, you need to be very careful. Writing to a database like that can cause corruption that could cause catastrophic harm to a patient. I don't know of any projects like that, and I would never use anything to write to a live database unless it came from the vendor and had been through the FDA approval process.
