Visa and Travel Preparation Meeting

Dear All,

We will be having the visa and travel preparation meeting on Tuesday, May 20, at noon in GEP.

For the students: please make sure you call PUSH at 765-494-1818. Give them an approximate departure date (I gave them July 26). They will schedule an appointment exactly 4 weeks before your departure. You can modify that appointment later based on your actual departure date. Only after you schedule that appointment they will be able to allow you to have the yellow fever vaccine early for visa purposes (you don't need an appointment for the vaccine). I will probably go Friday morning for the yellow fever vaccine (text/call at 765-412-9510 if you want to come along).

All the forms Mary refers to and the traveler information worksheet are uploaded in Google Drive (Purdue Hydro -> 2013-2014 Hydropower Team -> Travel_Summer_2014)

we will have the forms printed and ready for you to sign here at GEP, I want you to have time to read and prepare the answers
1) Bring your passports and yellow cards (if you have these already) on Tuesday
2) plan to sign the Travel Waiver Form-Student
3) you will complete application form - visa
4) plan to have your passport photos made here (free of charge) at GEP on Tuesday
5) plan to bring your money orders for the visa (see page 3 of the attached 'requirements' doc) - made payable to Embassy of Cameroon in Washington, D.C. in the amount of $124.00

6) after the google doc is uploaded, complete your information - ALL FIELDS ARE REQUIRED

Eleni, please upload the attached traveler information worksheet to the hydro team and ask each traveler to complete ALL information for him/her self - ALL info is needed in order to begin booking tickets.

Ideally, everyone will depart from Purdue campus and return here. However, I realize that it is summer and some of you may have already left campus, therefore, please indicate the city from which you will depart to join the team (in Chicago) and to which you will return. If from campus, just mark it WL
