In my previous life I worked for a company that manufactured hydraulic
steering systems for commercial marine vessels; everything from tugboats to
175' yachts. During installation, we always specified that the clevis MUST
be pinned to the cylinder rod. This was accomplished by drilling a hole
through the clevis and rod in the threaded area, then driving a small
stainless pin in to it. Lock nuts were not enough to stop the rod from
backing out of the clevis. We actually had a lawsuit as a result of
steering loss due to the clevis coming off, as yours did. The shipyard had
not pinned it, as specified on our installation drawings.
The rod and piston can, and will, rotate as it moves in an out of the
cylinder. Unless something prevents it.
