A few times in our discussions at hangouts and elsewhere, the subject of a musician registry has come up as a way for musicians to find each other. Also, in my role with the Creative Consortium of Pueblo, I've been working with the Greater Pueblo Chamber of Commerce and the Pueblo Convention Center, and it has become apparent that such a registry would be very useful when these or organizations want to hire musicians.
So I've started setting up a registry that could be used both for these purposes, and I'd love it if any of you interested would try it out and give me feedback before I start publicizing it outside this group.

the link to the form as I currently have it. I may make revisions based on your feedback, which might require you to resubmit, so please treat this more as testing than as an actual launch, and don't worry about accuracy as you fill it out.
The idea is this:
Any musician in Pueblo who wants to be listed fills out an online form with information like name, email, instruments played, etc. There is a place to choose whether you want your info to be shared with other musicians or not. This form then feeds into two versions of a spreadsheet - a private one with everyone's information, and a public one listing only the people who agreed to have their information shared. The public version will be made available to everyone who signs up. The private one will be accessible just to those looking to hire musicians. All of this will be completely free.
I tried to keep the form as short and simple as possible while including everything I thought would be useful for musicians looking to find other musicians and for people looking to hire musicians. If you choose to help test, let me know if there are places where you think the questions or choices provided seem unclear, whether you think I'm missing anything or asking anything inappropriately, etc. Also any feedback on the overall plan.
