PSRG Annual Dinner - RSVP by Wednesday

The PSRG Annual Dinner will be held on Saturday January, 7th 2017, 6pm at the Eggs & Us Cafe in Ballard starting with a social hour at 6pm and dinner starting at 7pm.  Despite the change in name, this restaurant is the same as last year.  Cost will be $26.00 which will be collected at the dinner. There will also be a cash bar.  We need to give the restaurant the final count of attendees and menu choices by Thursday morning so we need your RSVP/registration by Wednesday evening at the latest.  We cannot accept walk-ins.  Please RSVP via this form and fill in your name, call, email, and your choice for main course and dessert.  If there are multiple people in your party, please fill out a separate form for each attendee.
