PSM team: Currently we have a "Provider Statement" where the provider submitting the application certifies that they have answered accurately. Does that match what you're thinking of?
State: Yes. I suspect each state has its own rules to ensure an e-signature can with stand the legal impact.

That doesn't answer your specific question, but suggests that this will need to be configurable.CD
