issue with "add protein domain"

Hello, 

I am trying to add a customized protein domain and the "add protein domain" feature is not offering a box to input text for this feature. Is anyone else experiencing this issue? I've attached a screenshot. Is there a different way to customize protein domains?

I am using Chrome browser on a mac.
