Fasting treatment plan

Is this a treatment plan that you already have in place or consider implementing as a patient option?

Protocol: All patients followed similar dietary regimen. Patients 1 and 3 followed alternating-day 24-hour fasts, and patient 2 followed the triweekly 24-hour fasts schedule. On fasting days, the patients only consumed dinner, whereas on non-fasting days the patients consumed lunch and dinner. Low-carbohydrate meals were recommended when eating meals. Patients were examined on average twice a month and labs were recorded.

Results: The complete discontinuation of insulin in all three patients. All three were able to discontinue their insulin. The minimum number of days to discontinuation of insulin was 5 and the maximum was 18.

We have implemented various approaches to Intermittent fasting, but only patient has been involved in  full 24 hour fasts.  As a group we are learning about it. Cora has done several extended fasts.  Rachel has experimented on herself as well. There are clinical side effects that may not be good for working people.  Cora has taught us about an electrolyte supplement that is helpful.  We all follow Dr Jason Fung.

 

We can talk about this approach at our next meeting.  It will come down to motivation on the patients ‘ part as well as oral antidiabetic meds that make it risky- as we are the ones to hold these for patient safety- it is kind of risky in several ways.

Was Cora a complete fast? 
Could there be different levels of I.F. depending on patient choice and ability? 
I would like to see this option in the arsenal of therapies. Getting past the fear and apprehension of fasting is important. 
Patients need to understand that fasting, like overnight or spiritual, is natural and healthy, especially in those with diabetes and prediabetes. We want to dispel the myth of 6 small meals a day or 50 carbs per meal. 

How can this be applied to Vanuatu?

I did one long (5 days) a lot of 3 days, and a lot of 24 h. I will restart this week, and this time will have a spiritual component too. My target is 100 h. Can't see a better to start 2019.

How could we mitigate those risks?
There are several strategies for “fasting” including extended fasting, like Cora. 16/8; 23/1; and extended fasting. 
Westman used a fat fast with excellent results. This seems like what we all do at times is skip a meal or two but in this case it’s done with a strategic plan. Westman’s approach was to cut DM meds in half and adjust. In the article of three case studies T2DM insulin was deprescribed in 5-18 days. 

No question patients need to reduce diabetes medications, and that needs to be done with primary care provider help. In Vanuatu there are few physicians so that most of the care is being provided by village medics or nurses. 

I would like to see a fasting option in our program with management guidelines you feel are needed.
