What does this mean? Savannah, ten, CVID, has been having abdominal pain since early Sunday morning. Was unable to eat or play. By yesterday morning, she was holding her stomach and curling up on the couch, sleeping off and on, miserable. No fever, no vomiting, doesnt feel bad anyplace else, except that she complained off and on of severe cramping from the thighs to the ankles. I ended up taking her to the family dr, she didnt feel it was appendix or anything like that, but ran a urine sample and several labs. The urine sample, she said, had quite a lot of leukocytes in it, but she wasnt convinced it was an infection. The pain Savannah shows us is all around her belly button. Which seemed sort of high to be a UTI? But until the rest of the labs come back, she started her on abx, just in case. She's also got a really nasty reflux flare going on over the last several weeks, but Im not sure it related to that either.

The dr also spoke to me privately and said she feels Savannah is very depressed. I dont see it? I asked what makes her think that, and she said bc she is always very quiet, and unhappy in the office. My thinking is that bc when she sees the family dr, she's always SICK or in pain, and thats why she's like that. Other drs see her for follow ups, when she's not sick, and she seems fine. She did go through a huge depression and anxiety issue last yr, but I dont see anything like that going on. In fact, she is a fairly socially awkward child, but with lots of work, has tons of friends this yr, and is on the honor roll for the first time ever. I felt like things were really looking up. Now Im questioning if there were a problem, would I see it? or am I comparing everything with the horror that was last yr? I asked her if she had been feeling sad or unhappy, she said no, its just that her stomach really hurts. What do you guys think?

I am glad she put her on antibiotics!   I agree with you—the kids are different when they are sick.  If you don’t notice changes at home, I would not worry.

She is still on IVIG right?  Gut stuff can be a huge component of immune disease.  I could go into a long list of why, but suffice it to say, it just is.  Also, constipation can be increased with IVIG/SCIG and MANY kids with PIDD tend to be constipated kids.  For some, this can cause alot of mid gut pain.  BUT, it sounds like she definately has gut involvement with her PIDD.  It would not hurt to make sure she gets tons of water and I would talk with the doc about the possiblity of constipation.  For Maggie, too low a dose or complete lack of immunoglobin resulted in non stop tummy aches.  Now that we have her levels up again, she has not complained of a tummy ache once.
 
Good luck.  There is much more I would like to say.....but I want to stay away from giving medical advice.  Let us know how it goes please.

thanks:) she's not constipated, she's been "going", and dr checked her and didnt feel any hard stool. She does drink tons of water already, she's always thirsty. Anyway, its good we started the Bactrim, bc we were at pulmo today, and evidently she's got ANOTHER sinus infection starting. so now we'll finish the bactrim and we'll have to go back to prophylaxis for a few months. Her small airways are only up to 36%(26% at Jan appt), and thats a bit upsetting. She's lost more weight.....down to 59 lbs, at 55 inches, her clothes are falling off. Pulmo wants us to start her on dairy again and see how it goes. She's so picky(sensory issues) with eating, on top of no dairy/no eggs due to allergies, we're havinga really hard time getting enough calories in. Im going out tonight to buy Ensure and Boost......any thoughts on which one a VERY picky 10 yr old might tolerate?  And we'll be getting (another!!) call from the hospital nutrionist this week to work on some other weight gain ideas.
