Subject: PID Has anyone had?

Cross-posting here.....

Has anyone has cellulitis that has been resistant to Clindamycin?   If so, could you describe how the episode played out? Our Joseph now has his 8th or 9th bout of cellulitis.....

Recapping for those who do not know the story of our last few days.  Wednesday, Joseph was playing outside and got poked by a splinter of wood on his fort.  No blood, no splinter inside--no pain.  He kept on playing and did not say anything until 8 or so hours later when it hurt and was red.  I looked and the  puncture wound is the size of a pin head.  No abrasions anywhere else--just a small hole.   I didn't think there was any splinter in it.  Next day it has more of a red ring around it, we call ped.  By the time we get to ped, it has doubled in size.  She checks to see if there is anything in the wound and there is not.  Uses peroxide to clean it.  Circles redness again.. (I had marked it already from earlier and it was clear that it was growing)  Gives us a script for Clindamycin.

By the time we got home it had spread even more.  We get the first dose of Clindamycin in.... it continues to get larger.  Second dose in....and it continues until 3am to grow - slower than before starting the antibiotics.   This morning after his 8am dose...it seemed to be better @around 11am.  It was still outside the borders from where the ped marked it, but not growing larger.   Then, around 2pm or so, Joseph tells me that the redness is getting worse but that his pain is better.  I look and sure enough it has gotten larger!  I mark it with a pen again and call the ped (she had asked me to update her at some point today)

She agrees that it had gotten worse and says she expected it to get worse today...but if by tomorrow morning It is not back in the borders of where she marked it yesterday, we'll have to go to the hospital for IV antibiotics.    He had Clindamycin at 4pm and I just had Chris look at it and it is just slightly bigger than the line we drew this afternoon.  BARELY--maybe right at the line.

Usually he's shown some improvement with the cellulitis by the time we get to the 4th dose.....   Has anyone had it take this long to have the redness start reducing at least a little bit????

Guess we'll see what tomorrow brings.  We still cannot touch the site--he freaks out because it hurts so much.  He isn't limping as much--the pain has decreased.... But I still am bothered by the redness.   Just wanting to know what others have dealt with...and if anyone has had a bacterial infection resistant to Clindamycin.

Peace be with you,

Pattie

Are you certain it was a sliver?  It sounds as though he may have been stung or bitten by something.  What you describe is what happens to me when I get stung by a bee.  Once, I got into a hornets nest when pressure checking a fire hydrant.  My hand was on the bottom of the cap where I could not see it.  I felt a horrible poking sensation in my little finger and thought I was poked by the wire on the cap....the sensation was the same as a nasty splinter that nails you.  To make a long story short, there was no question about what it was after the entire nest swarmed and stung me 37 times before it was all over......and I am allergic to bees.  Anyway, if not a honeybee, I have the same reaction you describe......a sting that feels like getting stuck by a splinter (although I rarely see it happen).  Other than minor swelling the first few hours, nothing happens,  Then, within 24 hours, it blows up and gets bigger and bigger.  Without IV solumedrol and benadyl, it gets severe and I get a high fever with it.  Just a thought that perhaps it was not a sliver at all.  If not, then solumedrol might help if it does not start to resolve on it's own.  Some spider bites will cause the same sort of reaction.

Good Luck!

PS....Hi Mark!
