Thebirth registration team is responsible for registering live births filed by hospitals, midwives, or other individuals who deliver in Utah. A date of registration is assigned for each live birth certificate and any supporting documentation that is needed to register the birth certificate such as a paternity declaration. The registration team registers both current birth events and events that are older than one calendar year and have not yet been registered with the Office of Vital Records and Statistics (OVRS).

All live births filed with the Office of Vital Records and Statistic (OVRS) for individuals one calendar year or older shall be filed as delayed. Please contact the birth registration team at [email protected] or call 801-538-6105 to obtain further information on how to file a delayed certificate of live birth.


Even if you have never obtained a certified copy of your birth certificate, or know that you do not have a birth certificate on file, you will need to start the process by completing an application for a certificate of live birth. Order a Birth Certificate online or print a paper application. If applying by paper application follow instructions provided on the application, and along with picture ID and the required fee, mail the form into OVRS. OVRS will contact the applicant for further information and steps to be taken if a delayed birth certificate needs to be filed.


Utah law allows for the registration of early-term stillbirth deliveries for pregnancies between 16 - 19 weeks gestation as calculated from date of last normal menses period to the date of delivery. The request form requires a Utah licensed physician to sign. Follow instructions provided on the application, and along with picture ID and the required fee, mail the form into the Office of Vital Records and Statistics. If you're pregnant or have a child under five years old, you can get free, healthy foods from our Women, Infants and Children's nutritional program if your income qualifies. (If you receive TANF, Food Stamps or Medicaid, you automatically qualify for WIC).


By Mississippi law, a portion ($1.00) of the fee collected for each certified copy is deposited in the Children's Trust Fund administered by the Mississippi Department of Human Services to fund programs to prevent child abuse and neglect.


If you are applying for a birth certificate for yourself, a relative, or someone you legally represent, you must provide a copy of a valid photo identification with your application. Accepted forms of identification are:


Complaints of failure to receive certified records will be honored within 6 months of the original request. If the copy was to be returned to you by U. S. Postal Service, please allow 4 weeks after mailing the request before inquiring. Inquiry about copies ordered with payment for special courier delivery should be made within 7 days of the request. Mail returned because of insufficient address or address changes will be re-mailed if this office is notified of the correct address within 6 months of request.


Requests for vital records may be submitted to the IDPH Division of Vital Records via regular mail. Be advised that the processing time for requests is approximately 12 weeks from the time your paperwork is received. You will not receive an update from IDPH during the processing time.




In order to request a copy of a birth certificate you must complete the form Application for Search of Birth Record Files. To learn more, see FORMS located in the right-hand column. Completely fill out the form. Submit it with the following information:


A valid government issued photo identification (ID) that must be readable and is not expired/out of date (if an ID is not provided, unreadable, or expired, the request will be returned unprocessed).


For $15 for the first copy of a birth certificate and $2 for each additional copy of the same certificate requested at the same time, you will obtain a birth certificate with information collected at the time of your birth (information has varied throughout the years).


Note:

We can only issue one of each type of birth certificate for a person. For security reasons, this means that if you or anyone who is authorized (e.g. a parent/guardian) orders a replacement certificate, the previously issued certificate is no longer valid.


If you are applying for a birth certificate for a person who is deceased, you can only get a certified copy of birth registration if you are the next of kin, executor or estate administrator (supporting documents are required).


This certificate is recommended for newborns and children under 16 years of age where parental information is required. It can be used for the same identification purposes as a birth certificate with only subject information, but also for:


When a child is born in Ontario, their parent(s) or guardian(s) must register their birth with the Government of Ontario. A certified copy of birth registration is a copy of the original registration and:


You cannot apply for a birth certificate with parental information or a certified copy of birth registration unless the original birth registration has been replaced by a substituted birth registration. Please contact the Office of the Registrar General at 807-475-1171 or 807-475-1180 for information about an application for substituted registration on adoption.


If you give us a specific year, we will conduct a five-year search two years prior and two years after the year you specified. You can request searches for longer periods of time for an additional fee of $15 for every five-year interval searched.


These are two separate products, used for different purposes, with different information. Always check with the government agency or person requesting the document to confirm what information they need before ordering a birth certificate.


Note: Do not include credit card information in your application form. Once you have completed your form, you will be directed to a payment page where you can submit your payment information and complete your request.

The Office of Vital Statistics encourages individuals to mail in requests for vital records certificates. Complete the appropriate form and mail it to the Office of Vital Statistics located in the Jesse S. Cooper Building in Dover (see address on the form)


Under Texas law, voters who possess one of the seven acceptable forms of photo ID must present that ID at the polls when voting in person. Voters who do not possess and cannot reasonably obtain one of the seven approved forms of photo ID may fill out a Reasonable Impediment Declaration (RID) (PDF) at the polls and present an alternative form of ID, such as a utility bill, bank statement, government check, or a voter registration certificate.


All persons born in Hamilton County after 1882 have written birth and death certificates on file with the Health Department. Records are legal copies which can be used for all requests of these documents, including school registration. If you need a copy of these documents, please contact the Health Department office at your earliest convenience. Requests for these records are heavy during mid- to late August. To obtain a birth or death records via mail please select the proper application form.

The Health Department processes orders within 2 business days, excluding weekends and holidays, once all information is available. Newborn records may take up to 10-14 day to be available within the Indiana Department of Health vital records system for issuing.


You may order a certified copy of a birth, death or marriage record online using your credit card through our only approved official Internet business partner of the Tarrant County Clerk's office, VitalChek Network Inc.


Birth and death certificates may be issued to immediate family members--mother, father, brother, sister, parent, grandparent or child. While a grandparent may obtain their grandchild's record, a grandchild cannot obtain their grandparent's record without providing proof of a direct tangible need for the record. In addition, a certificate may be obtained by a legal guardian, legal representative of the family, and organizations or governmental agencies that have a direct tangible need for the record.


Contact the appropriate City Hall if the birth or death occurred in any of the cities during the dates listed below. Tarrant County Vital Records can only provide an abstract copy of these birth records and has no access to the death records.
