This email is for all clubs that had received DISTRICT SIMPLIFIED GRANTS last Rotary Year - 2011-12

to submit their reports in the form attached and copies of all receipts by 20 July 2012 to me.

Please complete all sections of the form and send it back as a word document.

Please do NOT send the report in other formats.
