The boy is 15.5 now (God I'm old) & we went to our AZ DMV. On the form, there was a check box that says "I want to show a medical alert condition on my license (must submit a physician statement)". I contacted our Endo, & here is his response: "I do not have a form or anything special related to this. Occasionally parents will request a note from me regarding wanting this to be listed on the license. There is no state requirement or form for this particular request."

What did you do for your T1D driver? I do want this medical alert on his license in case, God forbid, he is having a hard time driving & they don't beat him up thinking he's drunk... but at the same time I worry about labeling :(
