header, footer, lazy_bounding_box moving to prawn-layout

As the subject suggests, lazy_bounding_box, header, and footer will be
moving to prawn-layout, which means you'll need to use require
"prawn/layout" to use them in future versions of Prawn.
Keep an eye out for this change on edge, and adjust accordingly.

Don't say I didn't warn you. :)
