Whether I need to file any declaration along with the form.

 

Who shall give that decleration? (Applicant or professional)

 

If yes then please provide me the format of the same.

If the DIN form is certified by the professional, a verification from the Applicant needs to be attached to the Form DIN 1. The format is provided as Annexure I in the MCA website itself from where you have downloaded the Form DIN 1.
