DIN 4 for intimating PAN of director

Dear all,

 

Can anybody provide me the filed DIN 4 for intimating PAN of existing Director who has not submitted his PAN at the time of obtaining DIN.

 

Also let me know what to do, in case the name of Director (furnished in earlier DIN 1 form) does not match with the name of PAN which needs to be submitted in DIN 4.

 

Pls reply asap.

As far as mismatch of name is concerned, affidavit will suffice.

You can download the format of affidavit from MCA site.
