Hi All,

Apologies for the repeat if this already came through - I got a notification that the attached file was too large, so I am trying again with a compressed version.  Looking forward to hearing everyone's thoughts next week!

Dear Community Health Partners,

Thank you all so much for committing to attend our upcoming Community Health Improvement Plan Meeting.

Meeting Details
Date: Wednesday September 9th
Time: 5:30PM-8:30PM
Location: Choteau/Teton Public Library; 17 Main Ave N, Choteau, MT 59422; Alice Gleason Room

During this session, we will walk through key findings from the assessment, with an outside facilitator guiding our discussion. You can expect:

  • A structured conversation to reflect on the assessment findings
  • Collaborative small‑group work to identify priority themes
  • A brief action‑planning segment to outline initial next steps
  • Time for open reflection and questions
  • Dinner!

Prior to the Meeting
Attached is a DRAFT copy of our Community Health Needs Assessment.  (Please do not share this document further until we have time to finalize it.)
We would love to have you look through the data we have collected and think about the following questions:

  • What data points or findings stand out to you?
  • What surprises you?
No need to write anything down - just come with your thoughts!

Your participation is essential in helping us interpret these findings and determine meaningful priorities for our community. We appreciate the time and expertise you bring to this process.

If you have any questions ahead of the meeting, feel free to reach out.

We look forward to seeing you there.

Warm regards,
