None of these things have borne out in my experience. I always felt more awake, more present. I Was never as tired as with monophasic, especially during an afternoon slump. I didn't need coffee/tea, I was just fine all the time. 
Similarly my body didn't suffer. I've done weight training, marathon training, university, and work while also polyphasic, and nobody complained or worried about my health. 
The only negative I experienced after acclimation was that I was single, and asking a potential partner to just chill out for 25 minutes while I take a nap during a long date or an activity was very awkward and never progressed past the initial phases of the relationship.

LOL At the dates thing!  It's very hard to stop all the weird spilling out at once when that's the introduction.  

I don't know about looking older, but I definitely noticed my first grey hairs in my first years of Polyphasic.  But then again, I'm nearing 40, so that is probably not that surprising.  Keep exercising (especially weight-training) if your worried about your appearance.  A lot of aging is your muscles losing definition and becoming slack, and that has much more to do with your exercise than your sleep.

You should also feel good.  If you feel fine, you are fine, so get ur ass to the gym and you'll look fine too.

For long distance runs (marathon or ultra-marathon) - do you nap mid race?

Farthest I ever got was a 1/2 marathon, and napped before and after, so no big deal. I would have definitely napped mid race if I had been running that long.
