Quinine is FDA-approved for the treatment of uncomplicated Plasmodium falciparum malaria. It is not considered safe and effective for the treatment or prevention of leg cramps -- an "off-label" (non-FDA-approved) use. Quinine is associated with serious and life-threatening adverse events, including:
Thrombocytopenia associated with the use of quinine for the treatment or prevention of leg cramps includes:
Fatalities and renal insufficiency requiring hemodialysis have been reported.
Due to continued reports of serious side effects in patients using Qualaquin "off-label" (a non-FDA-approved use) for night time leg cramps, the U.S. Food and Drug Administration (FDA) has approved a risk management plan to warn against the use of this drug for such unapproved uses. Qualaquin should not be used for night time leg cramps.
Qualaquin use may result in serious and life-threatening blood-related (hematological) reactions, including serious bleeding due to a severe lowering of blood cells called platelets (thrombocytopenia), and a condition known as hemolytic-uremic syndrome/ thrombotic thrombocytopenic purpura which in some cases may result in permanent kidney damage. In some patients, adverse reactions result in hospitalization and death.
Due to the portal hypertension I have with my PLD, I've been taking prescribed diuretic medications for three years and those can sometimes produce painful leg cramps. My doctor has recommended several things:
1) Being diligent about the amount of water I'm supposed to drink each day (he gave me a guideline specific to my situation)
2) Reducing or eliminating non-hydrating fluids such as alcohol and caffeine
3) Eating at least one banana or orange a day for the potassium
4) Drinking 4 to 6 ounces of tonic water before bedtime. Modern commercial tonic water still has quinine in it, but not the dangerous amounts that were historically in the brew (see: http://en.wikipedia.org/wiki/Tonic_water)
My doctor specifically said not to take quinine tablets as those are considered dangerous.
