Parent and Players,
Although the snow has yet to fly it is time to register for Spring Soccer!! Registration Deadline is NOVEMBER 15th!!
Items needed to register:
1. Polson Soccer Registration Form
2. Medical Release Form
3. PYSA Player Information Form
4. Copy of Birth Certificate (for new players only -- prior players we should have on file)
5. Non-Refundable Deposit of $50 (Total Registration fee is $225 -- the balance will be due by January 15)

****PYSA is holding two registration nights this week where the forms and deposit can be dropped off****
LOCATION: Polson High School Gym
1. DATE/TIME: Tuesday, November 9 -- 7 to 8:30pm
2. DATE/TIME: Thursday, November 11 -- 7 to 8:30pm
If you can't make it to either of these registration nights you can mail the forms and your deposit to: PYSA, PO Box 991, Polson MT 59860
