Create and manage forms, surveys, and polls with form assets. As part of the Live Delivery Platform (LDP) module, form assets require LDP to be purchased and activated before they can be created in your site. Form assets also require a database to store form submissions.
Allows web visitors to upload a file that can then be downloaded from the form submissions. Set a file size limit; the maximum allowed by the system is 25 MB. You can also set whether certain file extensions are allowed or disallowed. A failure message for if the file upload fails is required, separate from the failure message for the entire form submission.
LDP Forms are designed to allow users to easily create surveys, volunteer sign ups, appointment forms, and other types of questionnaires. It is not recommended to use LDP Forms to collect sensitive information such as credit card or social security numbers.
The submissions list shows columns for submission date, ID number, and unless disabled, the IP address of the submitter. It also shows a column for each element and the submitted answer (if any), labeled with the Name of each element. Any files uploaded via the form display as a link; click the link to download the file. The file is accessible to anyone who has that download URL.
Click "Export CSV" in the top-right corner to download a CSV form with all the form submission data. Clicking "Delete All" permanently clears the submissions, and cannot be undone. It will also delete any files uploaded via that form from S3 storage.
Each form asset element has an "Advanced" field. Adding attributes into this field defines additional properties for the questions, such as datasets for possible answers and the size of an answer field. Attributes are always lowercase and in certain cases, more than one attribute can be included in the field. For them to function, however, these attributes must already be defined either within the XSL or CSS of the site; the default state of Omni CMS does not necessarily have them defined.
Defines the opening tag for a tag within the form. This is used in conjunction with fieldset_end to set off a group of elements. It can also be used with fieldset_label to define a label for the group.
Adds a size attribute to a single-line text field within the form. The size attribute is used to specify the viewable size of the field defined in characters. This does not determine the number of allowed input characters.
Adds the cols attribute with the specified value to a multi-line text field form element. For HTML output this attribute specifies the visible width in average character widths. This does not determine the number of allowed input characters.
Adds a rows attribute with the specified value to a multi-line text field. For HTML output this attribute specifies the number of visible text lines for a textarea. This does not determine the number of allowed input characters.
My office recently updated their computers and when doing so we lost all our capability to read and use any of our previous forms created in OmniForm. I just need a away to download the program so I can retrieve all previous forms before the our company update.
To retrieve previous forms created in OmniForm, you will need to reinstall the OmniForm program on your updated computers. You can try contacting the OmniForm software provider or checking their official website to download and install the program.
The product is not available in the disc form to purchase. The purchase will grant you access to the downloads which you can extend this access period by adding another option to the purchase cart. There is no 5.1 version available. The only update refers to version 5.0 and it enhances the driver. The price is set to $99.99.
I'm looking for a program like OmniForm in which you can scan a patient's medical form and enter it directly into the computer database while allowing for format changes and corrections, but I need this program to be taken a step further.
I need the program to also be able to take that data and export it into a premised document. Like inserting the answers from the questionnaire into anm already formatted text document (like a fill in the blank paragraph).
There is an alternative from the RJS Software but in order to establish exactly if their products suit your business, a direct contact needs to be established. Access the official website and then fill the form. Explain the situation and they will contact you back with all the available offers and if it's possible to use their product for your business.
No, the software can't be used on Windows 7 Professional since Nuance discontinued the application. You will have to use another software called FormDocs which does the same thing Omniform is doing and besides that, it's still available. There is a support page that was supposed to offer information about Omniform but all the links leads to 404 error pages.
Omni Forms can be placed inside a page using an Asset (reusable icon in your WYSIWYG toolbar). Assets are designed to be reused across multiple pages while the asset remains the same. So, if you are using a form that needs to appear in multiple places across your site, you will need to place that same Asset in multiple places. Likewise, if you wish to have a unique form across multiple pages on your site, you will need to create a unique form each page using Assets.
4. Name and edit your Form fields as needed by dragging and dropping the elements that appear in the left-hand column into your form.
Various options, included required field, can be selected in each form element.
5. IMPORTANT - Be sure to adjust your access to your form to just the directory (site) you intend to place your form.
By default, "Everyone" is selected so if you do not wish for other site authors to have access to your form, you must select your directory.
6. Be sure to adjsut your email setttings as needed under Email Messages. This is where submissions will be sent if you wish to receive an email when a form is submitted.
You will be able to collect data (form subissions) by revisiting this asset at anytime in the future.
8. After checking the page out for editing, locate the editable content region where you wish for your form to be placed. Ensure your page contains an appropriate Snippet so your form aligns properly on your page.

FormDocs is an electronic form design, form filling and forms management software similar to Omniform, but with on-going development and support for over 500,000 users worldwide. There are two (2) steps. The first is to convert the Omniform form design to a FormDocs form design, and the second is to export Omniform data to an ASCII text file and then import that data into FormDocs. The result is a FormDocs form file having the same look and feel and data content as it did in Omniform.
If you are still able to run Omniform then you can install the FormDocs Conversion utility on the same computer (i.e. you will have Omniform and Formdocs installed and running on the same computer). The FormDocs Converter will do its best to convert the Omniform form design to a FormDocs form design. The result will be a form design file that you can 'touch-up' and edit in FormDocs Designer. Similar to Omniform Designer, the FormDocs Designer is the application in which you can create and edit form designs.
