Elevate provisional items and replace macros

Hello all

I have submitted an RFC to update the Standard per issue #518 (https://github.com/pmix/pmix-standard/issues/518). This involved:

* elevating all provisional items to stable EXCEPT for the storage chapter

* replacing all macros with their functional equivalent, removing the macros from the Standard. Exceptions were the static initializer macros as those cannot be functions. The replacement functions are marked as provisional.

* update the Revisions chapter to track it all

Along the way, I found several copy/paste errors in the text, so I corrected them. Pretty minor stuff - e.g., citing the wrong data type. You’ll also see some minor whitespace deletions at the end of lines because my editor does that automatically, and a few places where I standardized a function declaration to the “new” style.
