Thebirth registration team is responsible for registering live births filed by hospitals, midwives, or other individuals who deliver in Utah. A date of registration is assigned for each live birth certificate and any supporting documentation that is needed to register the birth certificate such as a paternity declaration. The registration team registers both current birth events and events that are older than one calendar year and have not yet been registered with the Office of Vital Records and Statistics (OVRS).
All live births filed with the Office of Vital Records and Statistic (OVRS) for individuals one calendar year or older shall be filed as delayed. Please contact the birth registration team at [email protected] or call

Even if you have never obtained a certified copy of your birth certificate, or know that you do not have a birth certificate on file, you will need to start the process by completing an application for a certificate of live birth. Order a Birth Certificate online or print a paper application. If applying by paper application follow instructions provided on the application, and along with picture ID and the required fee, mail the form into OVRS. OVRS will contact the applicant for further information and steps to be taken if a delayed birth certificate needs to be filed.
Utah law allows for the registration of early-term stillbirth deliveries for pregnancies between 16 - 19 weeks gestation as calculated from date of last normal menses period to the date of delivery. The request form requires a Utah licensed physician to sign. Follow instructions provided on the application, and along with picture ID and the required fee, mail the form into the Office of Vital Records and Statistics. Note:
We can only issue one of each type of birth certificate for a person. For security reasons, this means that if you or anyone who is authorized (e.g. a parent/guardian) orders a replacement certificate, the previously issued certificate is no longer valid.
If you are applying for a birth certificate for a person who is deceased, you can only get a certified copy of birth registration if you are the next of kin, executor or estate administrator (supporting documents are required).
This certificate is recommended for newborns and children under 16 years of age where parental information is required. It can be used for the same identification purposes as a birth certificate with only subject information, but also for:

If you give us a specific year, we will conduct a five-year search two years prior and two years after the year you specified. You can request searches for longer periods of time for an additional fee of $15 for every five-year interval searched.
These are two separate products, used for different purposes, with different information. Always check with the government agency or person requesting the document to confirm what information they need before ordering a birth certificate.
Note: Do not include credit card information in your application form. Once you have completed your form, you will be directed to a payment page where you can submit your payment information and complete your request.

Under Texas law, voters who possess one of the seven acceptable forms of photo ID must present that ID at the polls when voting in person. Voters who do not possess and cannot reasonably obtain one of the seven approved forms of photo ID may fill out a Reasonable Impediment Declaration (RID) (PDF) at the polls and present an alternative form of ID, such as a utility bill, bank statement, government check, or a voter registration certificate.
You may only accept an original or certified copy of a birth certificate issued by a state, county, municipal authority, or outlying possession of the United States that bears an official seal. Versions will vary by state and year of birth.
Our processing times for complete paper applications through the mail are four weeks from the time they are received in the Office of Vital Records. Our online application processing times are two weeks from the time they are completed and submitted. Records requiring amendments will take longer depending on the nature of the change.
If you need your records faster, you are strongly encouraged to utilize the Express Delivery option or to check for walk-in locations near you for current hours of operation. Our main office at 2001 Maywill St in Richmond, Virginia 23230 is operating our public lobby Monday - Friday 7:00 AM to 4:30 PM.
The Code of Virginia requires a cost of $12.00 per certificate search. There is no refund if the certificate is not found. An exception is a Certificate of Birth Resulting in Stillbirth, which have no cost. A fee of $10 is charged for making an amendment to an existing record.
The Wisconsin Vital Records Office has closed our in-person services. Vital Records services, including requests for birth, death, marriage, and divorce certificates, are available by U.S. mail, online through VitalChek, or by phone through VitalChek at

About two weeks after the birth of a child, the mother will get a Notification of Birth Registration form in the mail. You can use this form to make corrections to and order copies of the birth record.
If the death happened less than one year ago, corrections can still be made by the funeral director. This includes changes to name, date of birth, marital status, address, etc. If changes must be made to the cause-of-death section, they must be made by the medical certifier.
The names on the CRBA include the parents who have a genetic or gestational connection to the child. The parent(s) passing U.S. citizenship to their child must have their name(s) on the CRBA. A parent passing U.S. citizenship may approve the other parent, who is not passing citizenship, to have their name on the CRBA.

If one parent is not a U.S. citizen or if the U.S. citizen parent passing citizenship to your child is not present, that parent who is not present should complete Form DS-5507. Use this form to list the periods of time the parent, who is not present, spent in the United States.
If your child was born out-of-wedlock, and the father is a U.S. citizen or non-citizen U.S national, use Form DS-5507. The father of the child must sign Form DS-5507 at a U.S. embassy or consulate, or in front of an official who registers births or gives oaths.
We are working to make the X gender marker available on Consular Reports of Birth Abroad (CRBAs). Please note we are currently offering customers the option to select an X gender maker on a U.S. passport.
If you got U.S. nationality when you were born in one of the U.S. territories listed below, you are not eligible for a CRBA. You are not eligible because you are not considered to have been born abroad.

The Missouri Department of Health and Senior Services, Bureau of Vital Records, is designing a brand new Missouri Electronic Vital Records (MoEVR 2.0) system with a phased implementation period occurring over the fall of 2024 to the spring of 2026.
