Dear girls, this email came to me this week, it is about Ryan going to school. I would like to help them so I need your help. At the moment he is most of the time with Laura and Niamh. But he used to be with Anna and Ozge as well. Could you please have a look at the questionnaire and choose one of you to fill in this form, please. My name is Rebecca and I work with a company called Servisource Healthcare. We are currently working with the HSE to conduct Assessments of Need in Galway. Part of the ASD/AON assessment process is to get an up-to-date school questionnaire/interview completed. 

  

One of the referrals we have received is for R.O’Rourke (DOB: 06.01.17). His parents passed me your details. Would you mind completing the attached form and returning it back to me at your earliest convenience? The questionnaire can be returned via email, if you would prefer to post please let me know, and I can provide the address.
