Document couldn't be opened

I tapped on a file, and above its title was written "Document couldn't be opened." There was no text in the document, and I couldn't tap to bring up the keyboard. I tried to email it to myself to see if it would show up, but it didn't. As I tapped "share," "Document couldn't be opened" was replaced with "Document couldn't be saved." So I looked up any possible solutions, found none. I checked the app on my phone, same thing. When I went back to it on my iPad, it no longer said "document couldn't be opened." It was just a blank document. I would like to know what happened to this file, and why the entire thing got deleted.
