Patient Medical Record # matching between Pinnacle and Mosaiq

Dear List.
 
Our CT Tech accidentally put the wrong Medical Record # on the CT image. I changed it after I loaded it to Pinnacle. But I still can't export CT image to Mozaiq from Pinnacle. I had to change the MR# in Mozaiq so that I can export. My understanding is that I can export to Mozaiq after I changed the MR# in Pinnacle even the MR# from CT is not right. Anyone experienced same problem? Is that R&V related. I used Lantis before. Looks like I had no this problem if I export to Lantis. of course I didn't export the CT image to R&V since Siemens machine doesn't relay on R&V for the CT image. Thanks. I can export the plan to Mozaiq even with wrong MR#.

The problem lies in exporting the Cone Beam CT to Mosaiq as now the image set does not match the plan. Changing the patient’s MR number is never a good solution to this problem and in many institutions is not possible.

 

Below is a work around that I’ve put together with the help of Philips. This assumes that a “copy” of the plan is created for CBCT export which has only the relevant structures included.

 

I hope it helps.

 

If the planning CT is sent to Pinnacle with an incorrect Patient ID # entered and it is not noticed before planning is completed, the CT images will not transfer to Mosaiq.

The following process was found to be an effective work around to this problem while avoiding the need to re-plan.

 

1)      Ask the RTs to re-send the CT dataset from the scanner with the correct MR#.

2)      Create a new plan using the newly imported scan as the primary dataset.

3)      In the patient select window, click “edit” under the patient name.

4)      Click “revision history”.

5)      In the upper right-hand of the window, place the cursor in the “Directory” window.

6)      The displayed path will look similar to this: /pinnacle_patient_expansion/NewPatients/Institution_4/Mount_0/Patient_1488

7)      Using the arrow keys, move the cursor all of the way to the right.

8)      Record the Institution number, Mount number, and Patient number.

9)      Now open the new plan created in step 2.

10)   Go to the contours tab>Options>ROI Spreadsheet

11)   In ROI Spreadsheet go to File>Import ROI

12)   Place the cursor in the “Directory” window. Select the displayed path and delete.

13)   Enter the following path: /usr/local/adacnew/Patients/Institution_4/Mount_0/Patient_1488 (substituting the Institution, Mount, and Patient numbers recorded in step # 8)

14)   Select the appropriate plan (plans will be labeled as “Plan_0”, “Plan_1” etc… so select the plan that corresponds to the “CBCT” plan that you’ve created for this patient. ie. The second, or third plan in the list)

15)   In the left-side pane select “plan.roi”

16)   The new plan will now populate with the “CBCT roi’s” and will give an error message for each. This is normal so click through these.

17)   Now open the “Expansion/Contraction” window and expand each roi by “0”. To do this select each structure in turn, select “Create new ROI” and type the name of the structure you’re about to expand but include an “x” on the end of it. This will effectively “fool” Pinnacle into believing that these structures were created on this dataset.

18)   Once all structures have been “expanded by 0” into new ROIs, delete the original ROIs and remove the “x” from the end of the names for the new ROIs.

19)   Reset the colors for each structure.

20)   Add a beam to the plan. This will also generate a POI, and a prescription. These are needed in order to export.

21)   Export as usual.

Have.ct.resent.with.correct medical record.number.so.it.overwrites data in.pinnacle. Then.it.will.go.to.Mosaiq

What I do is just send the corrected image set to Mosaiq using DCMTree.  You don’t even have to put it in Pinnacle.
The structures sent from Pinnacle to Mosaiq line up because the UID of the image set is not changed when you change the MR #.
DCMTree is a program that is in your IMPAC directory.

the easiest thing. I would guess, would be to change MR# in mosaiq to what was originally used by CT tech when CT was taken. Export CT series from Pinnacle to Mosaiq. Change MR# in Mosaiq to right ID.

Thanks very much. I did the way you said here. I had to change back for CBCT since the image system wouldn't recognize the patient if I don't change back to the right MR#.

that is correct, the MR# in Mosaiq is switched back to correct one immediately after CT image import from Philips. We also open the CT image immdediately and change the status to Review Not Reqired.
