Hi everybody,

 

I’ve a problem for a patient. For breast cancer we modified patient orientation when we load it on Pinnacle after what we make dosimetry on it.

Today we have to send this patient to another center but Pinnacle don’t allow us to send RTStruct and RTDose if patient position was modified before.

I’ve tried to go on the mount_0 space and on the patient folder. I’ve found some files like “plan.roi”….. but it’s not in .dcm extension.

Is there anybody know how to found RTStruct and RTDose on Pinnacle in this case?

Hi

That's a safety feature of Pinnacle, because Pinnacle can't guarantee the orientation is correct.  I've looked into orientations on Pinnacle to allow us to deliver eg prone plans on supine phantoms, but it's not a trivial problem (and I'm glad of that - I would not want to risk mistreating a patient!).

Pinnacle doesn't store objects in DICOM format, it builds the DICOM files when you export in DICOM format.  The doses are stored in files, each one is an individual slice of the dose distribution, and ROIs are stored in other text files.  Converting these to DICOM isn't something I've done, and I couldn't do it quickly.  Maybe a student project, unless someone has done it before?  But you need to be careful you don't make a mistake in the orientation in the DICOM files - remember, the CT files have to have the correct orientation too, and your OIS will almost certainly perform some validation checks (but you may end up with the wrong geometry at the linac).

My opinion is that you need to replan the patient without changing orientation, and test it on the linac prior to patient delivery.  Or just rescan the patient with the correct orientation and replan.

I’m ok with you for the safety but we’ve treated this patient 5 years ago and now she has another cancer and we have to send all informations about this old treatment.

We can’t rescan or replan patient in this view that’s why I just want to try to convert Pinnacle proprietary format to RT format.
