Roxen restart or reload causes many modules to crash aleatory

Hi pike team.
I would need someone who is a roxen webserver guru to tell us how to
diagnose a case that is causing us a lot of problems in the company.
We have been using roxen for many years, but lately the new versions
present a problem that we do not know how to diagnose.
On a server where we have several websites, when restarting roxen, we
notice that some modules randomly load with error.
The same module can load correctly on one site and incorrectly on
another, all very randomly, and without no apparent reason.
For this reason we would be grateful if you could give us some
guidelines to detect the cause of the problem or maybe there is someone
pike user that knows the cause.

I assume Roxen 6.1.246 or earlier.

This sounds like bug 7783/WS-141. Have you applied patch

Hi Henrik,
thanks for your answer.
As you can see on the image, we have a Roxen 6.1.200 running
with all patches installed ( also included the 7783/WS-141 )
The OS is a Debian 8
Any other idea?
