MEDICAL CERTIFICATION REQUIREMENT

OBJECTIVES

• To ensure that the SN is in excellent condition before going on a traineeship
• To detect any illness present (even if the SN might not feel sick) that might keep the SN from pursuing the traineeship
• To detect any illness that might pose as a danger to the SN if s/he pursues the traineeship

PRE-TRAINEESHIP MEDICAL EXAMINATION INCLUSION

• Complete Physical Examination by a Physician

- Medical History

- Oral Examination

- Visual Acuity

• Chest X-Ray
• Complete Blood Count (CBC)
• Routine Urinalysis and Fecalysis
• Drug Screening (Marijuana & Shabu)

B. Terms and Conditions

      Results will be submitted after three (3) working days from the time the examination and/or tests were done

Mechanics: 
IF APPLICANT PAYS TO CLINIC:

1. A referral form will be issued to the COMPANY. The COMPANY will print this, place name of applicant and sign. This will be given to the applicant and submitted to the clinic on the day of examination.
2. Classification (fit or unfit) of the applicant will be emailed to the COMPANY one (1) day after the examination.
3. Hard copy of the confidential medical report will be delivered to the COMPANY after three (3) working days.
4. Payment shall be charged cash to the examinee.

As of August 30, 2005, Healthway Medical Clinics, Shangri-La branch, will be providing medical services for our SNs. It is located at the Wellness Zone, near American Eye, 5th floor, Shangri-La. I also gave the contact person (Ms. Louie Ramos) Halmen's designation and mobile number.

  For SNs:

1. Name ESN Name
2. Company EAIESEC Philippines Foundation Inc
3. Always check the box marked: Pre-employment Exam
4. Authorized Signature Eanyone from the MC&NST
5. Always check the boxes marked: Charged to patient and Released to company
6. We shall require the SNs to have their Medical Examination taken at the Shangri-La branch only. If possible, in one day sabay-sabay na sila, preferably one Sunday because the clinic is full every Saturday. The reason why they all have to go there together sana is that they will be signing a waiver form (required by the DOH) kasi before undergoing Drug Screening. Since we want to be discreet about the drug screening, para hindi sila magsabihan, they should all have it in one day.

For other AIESECers:

We can also use this referral form for personal reasons. We still have to pay though but at discounted rates. Under 'Authorized Signature Eagain anyone from the MC&NST.   I am also attaching the Doctors ESchedules in all of their 6 branches. We AIESECers can use this form in any of these branches: Alabang Town Center, Shangri-La Plaza, Greenbelt, Festival Mall, SM North, and Robinson's Place. It's better to have an appointment (1) to confirm doctor's schedule (in case there are changes) and for (2) less waiting time.
