Add Extra Field in login screen using pgina. username, passowrd and OTP

I need to add an extra field to login. just need a text field. no authentication or purpose for that field is required right now. just a modification in interface. i need to know how i can achieve this for windows using pgina. do i need to create a plugin? or anyother way to achieve this.
