Good Afternoon Team,

During the Lab Meeting on Tuesday 10/02/18 we were advised that it is now a requirement to have two forms of identifiers on the Wet Mount Log. Please see the attached log that needs to be printed and added to your log book, so that all Wet Mounts from today on will show patients MR# (Medical Record number) and Date of Birth. I will deliver them in person to the Davie Clinics.
